Job Summary / Purpose

Work with patients in the clinic setting to perform evaluation and management of sub-acute and chronic illnesses.

Essential Key Job Responsibilities

1.      Obtain medical and developmental history as appropriate.

2.      Perform comprehensive or focused physical exams.

3.      Order appropriate screening and diagnostic studies.

4.      Prescribes medications and treatments, as indicated.

5.      Provides patient and family education and instructions regarding medications, treatment plans, and health maintenance.

6.      Consults / collaborates with physicians and other allied professionals.

7.      Determines if referrals are needed.

8.      Keep complete and accurate records in the patients chart.

9.      Assures that adequate patient health records are maintained and transferred as required when patients are referred.

10.  Participate in the development, execution and periodic review of the written policies governing the services the clinic furnishes

11.  Participates with a physician in a periodic review of the patients’ health records.

12.  Complies with Hospital policies & procedures, as applicable.

13.  Complies with regulatory agency standards.

14.  Performs other related duties as assigned or requested.

Required Education (for CHI Leadership Job Levels from Supervisor through President)

Medical Degree from an accredited medical school is required.

Must have successfully completed a residency program.

Two-plus years of experience in field of practice is preferred

Required Licensure and Certifications

Must be board certified and possess a current non-restricted license in Oregon. Must possess current, valid federal and state narcotics certificates. Current BLS certification required

Required Minimum Knowledge, Skills and Abilities

Ability and willingness to demonstrate and maintain competency as required for job title and the unit/area(s) of assignment.
