Job Summary / Job Purpose


The Patient Flow Representative: will work with minimal supervision and is responsible for coordinating transfers, direct admissions, bed assignments and throughput in all KentuckyOne Inpatient facilities. The Patient Flow Representative will proactively identify and manage throughput barriers and implement a resolution strategy. The Patient Flow Representative will collect and prepare needed data for the Access Center department and Management Stakeholders. She/he will deploy Clinicians to evaluate patients within eligible facilities. Must coordinate and prioritize routine patient needs and emergent patient transfers/placement. Will work with House Managers and Staffing Coordinators to guide contingency staff to areas with increased placement demand. This position must be able to function independently, organize, prioritize, work well under pressure, and deal effectively within all levels within and outside of the organization; including, but not exclusive to Physicians, Providers, and referring facilities.


Patient Flow Representative:


Validates accuracy of staffed beds, patient requests, and entered data in all applications within Transfer Center and Capacity Management Suite of Teletracking.


Arrange, create cases, and requests for Transfers and Direct Admissions into the KentuckyOne Health system according to appropriate placement and admission protocols, both system-wide and facility specific.


Follows escalation procedures for day-of placement issues or critical placement needs, as well as delays of Physician response and Acceptance.


Reviews planned admissions/placements with Facility Leaders and status of upcoming challenges.


Report to Leadership on effectiveness, use, and compliance of throughput software.


Performs transfer in accordance with applicable laws and regulations (EMTALA).


Validate appropriateness of safe and timely transfers through screening and escalation.


Communicate effectively and concisely with incoming patients, physicians, co-workers, and all clients that call the KentuckyOne Access Center.


Coordination and documentation of emergency transfers via air medical service, internally and externally


Maintains confidentiality of patient information in compliance with HIPPA and KentuckyOne Health Policies.


Scan/copy/produce protected health information as needed for safe and timely patient placement.


Run routine reports to identify gaps in use and best practices of the systems.


Must be able to navigate multiple computer programs, transcribe notes while providing exceptional customer care via the telephone, and have the capacity to learn other relevant systems and databases as needed.


Facilitate nurse to nurse report for patients transferring to the KentuckyOne Health system as needed.


Demonstrates a commitment to a high level of professional and ethical standards in her/his daily activities, consistent with the KentuckyOne Code of Conduct and Mission and Values


Performs other duties as assigned

Job Requirements / Qualifications 


Education / Accreditation / Licensure (required & preferred):

High school diploma or equivalent required


Requirement 4 years of applicable experience without a bachelor’s degree, 2 years of applicable experience with a bachelor’s degree.


BLS certification preferred


Experience (required and preferred):

Experience in Telecommunications preferred


Exceptional customer service and conversational skills is required


Excellent written and oral communication skills is required


Experience with Excel, Word and PowerPoint preferred


Experience running and understanding reports is preferred


Patient registration experience preferred.


Knowledge of medical terminology and patient placement required


Minimum 2 years acute care setting required


Excellent oral and written communication skills required


Excellent Excel and Word skills required. 


Must have attention to detail and maintain a positive working relationship with peers and managers
