The home visitor provides culturally sensitive home visitation services to parents and/or families located in Northern Navajo Nation / Cuba, NM area beginning at pregnancy to the age 3 of a first born child, based on program curriculum and fidelity guidelines.  Works closely with St. Joseph’s Children (CHI SJC) Enhanced Referral Services Program staff to insure successful connection of children/parents/families of home visiting program with needed resources and services.

1.  Conduct home visits weekly and schedules visit times to meet the needs of families and the program (which may include some early morning, evening and weekend visits as approved by supervisor).  Hours may vary and Home Visitor must be flexible and accountable for managing time daily in an efficient and effective manner.  Meets CHI SJC program goal for number of home visits weekly as assigned by Program Director and Team Leader. Provides regular shadowing / training opportunities for newly hired staff in the program. 

2.  Achieve competency in all training protocols.

3.  Follow program curriculums (First Born and Partners for a Health Baby) and guidelines established by the curriculums and CHI SJC, including CHI SJC’s Core Values and religious and ethical guidelines.

4.  Maintain confidentiality of information at all times; obtain appropriate release of information as needed.

5.  Connect parent/family with CHI SJC Enhanced Referral Program as applicable.

6.  Appropriately represent CHI SJC in all interactions. Completes outreach assignments and outreach events as determined by the Team Leaders and Program Director.  Outreach assignments are completed during working hours but can also include weekends as needed.

7.  Maintain family case records in the database: Insures that all notes, assessment data, and other data base entries are accurate and up-to-date and entered within 5 working days of the home visit, and provide timely and accurate reports.

8.  Effectively meet needs of parent/family and child, respecting strength and preferences of parent/family.

9. Acts and serves as an Advocate for young children and families.

10. Completes other duties as assigned by Team Leader, Program Director, and/or President.



Knowledge, Skills and Abilities:

  • Fluent in Diné language strongly preferred.

  • Achieve competency in all protocols, curriculum, record-keeping, database use.

  • Perform duties within approved scope and in keeping with CHI SJC values and religious and ethical directives.

  • Demonstrate excellent interpersonal communications skills.

  • Ability to remain calm in difficult situations.

  • CPR Certification (attain within 6 months of hire)


Required:  A minimum of two years experience working with a pregnant women and/or families and/or childcare setting.  Ability to maintain accurate records and correspondence. 

Preferred:  Prior home visiting experience.


Required:  Must have reliable transportation; clean driving record, current NM driver’s license and current liability insurance.
