Job Summary:

This job is responsible for participating in development, management and evaluation activities relating to the grant-funded Highline Health Connections (HHC) program which is designed to provide a community-based care management transition model that will support the most challenged, challenging, complex and vulnerable patients.  An incumbent partners with a wide variety of internal, external and community-based agencies and work teams to link the most vulnerable underserved populations to evidence-based, cost-efficient care/support, toward achievement of goals focused on reducing overutilization, connecting patients to appropriate levels of healthcare and community resources, and coaching/supporting patients in achieving their personal health goals.

Essential Job Duties:
Researches and participates in developing and administering grant application and supporting documentation to maintain funding and/or obtain additional funding for the HHC program to facilitate achievement of stated program objectives.
Plans, manages and evaluates daily provision of care transition services designed to improve healthcare outcomes for enrolled patients, reduce healthcare costs/overutilization and bridge gaps/connect patients to appropriate levels of healthcare and community services; ensures that all activities are conducted in a timely manner and in accordance with all applicable requirements.
Plans, supervises and evaluates the work of assigned clinical and support staff.
Develops and maintains effective relationships with CHI/CHI-FH and community-based agencies/resources to ensure the HHC program is effectively integrated with overarching strategic plans for care transitions programs.
Maintains knowledge of current developments in the care transitions/management/coordination healthcare field and federal/state regulatory requirements impacting program coordination/operation
Performs related duties as required.

Education/Work Experience Requirements
Bachelor’s degree in a directly related health care field and five years of progressively responsible clinical work experience that would demonstrate attainment of the requisite job knowledge/abilities.  If licensed as a Registered Nurse, requires graduation from an accredited school of nursing.


Preferred Licensure:
• LICSW - Licensed Independent Clinical Social Workers); OR
• LASW – Licensed Advanced Social Worker, OR
• RN - Registered Nurse
