Job Summary / Purpose

The Community Health Worker will work primarily within the healthcare provider offices as well as with Emergency Department, community, at client homes, hospital case managers, and with other agencies/organizations to coordinate care and client needs.  Works as a team member focused on care coordination.

Essential Key Job Responsibilities

1.      Conduct health education and screening outreach in community and healthcare provider settings in order to provide health education and raise awareness of health concerns and available community resources and services.

2.      Reduce client barriers to health care and needed community resources and services.

3.      Facilitate communication and coordination between community resources/services and providers.

4.      Maintain knowledge and understanding of available community resources, services, and programs.

5.      Create connections between vulnerable populations and needed community services and resources.

6.      Provide health screenings for conditions and diseases as well as providing health information on those condition/diseases and areas of concern.

7.      Assist with establishing a unified referral system, as well as evaluate and improve the community health workers program.  

8.      Works closely with Providers and care coordination teams.

9.      Help patients develop health management plans and goals.

10.  Coach patients in effective management of their chronic health conditions.

11.  Ability to effectively communicate to staff and patients.

12.  Thorough knowledge of the meaning and use of medical terminology and abbreviations.

13.  Cultural Competence

14.  Ability to work collaboratively, Basic understanding of anatomy/physiology and clinical procedures.

15.  Willing to become certified as a community health worker and maintain knowledge in the following areas: Medicare Oregon’s Health Insurance Marketplace (“Marketplace”), community resources and services, duties/roles of community health worker/and or non-traditional home workers.

16.  Demonstrate sound judgment and composure with the ability to take appropriate action in questionable or emergency situations.

17.  Maintains a positive, caring attitude towards patients.

18.  Good computer skills using a medical management application.

19.  Possess a strong work ethic and a high level of professionalism

20.  Complete, when appropriate, evaluation in the home.

21.  Other duties as assigned by management.


Minimum Qualifications

Required Education (for CHI Leadership Job Levels from Supervisor through President)

High School Diploma or equivalent.

Basic Life Support (BLS) or Advanced Care Life Support (ACLS) required.

Required Licensure and Certifications

Community Health Worker Certification (to be obtained within the first 3 months on job)

Required Minimum Knowledge, Skills and Abilities

Language Ability: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure, manuals. Ability to effectively present information and respond to common inquiries or complaints from groups of managers, clients, customers, and the general public. Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compare rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety if instructions furnished in written, oral, diagram, or schedule form.

Computer Skills: Must be proficient with the Microsoft Office Suite, Internet navigation, database management, spreadsheet, E.H.R., and graphic presentation packages.
