Job Summary:
This job is responsible for providing general office assistance and support to facilitate the administrative aspects of care coordination, discharge planning and utilization review in accordance with established standards and procedures.  Work is directly focused on supporting the clinical care management team at the assigned hospital to maximize effective discharge planning that provides for patient safety and “best practice” quality and continuity of care.  An incumbent implements administrative aspects of patient discharge plans prepared and delegated by social worker or nurse case management team members, including coordinating activities/details with home care agencies, post-acute care facilities, durable medical equipment companies, transportation agencies and others as indicated.  Work involves active communication with patient/family and all members of the healthcare team on tasks assigned, as well as communicating and documenting follow-up activity in the medical record.


Essential Duties:
Provides administrative support in the coordination of general discharge planning and utilization management (UM) activities as directed by the Care Management team; handles incoming and outgoing communications, answers questions and provides information in accordance with established procedures.
Provides administrative support in making arrangements for patient post-acute care services. 
Prepares and/or maintains timely and accurate documentation of all activities in accordance with established standards and procedures; enters data in appropriate care management platforms (e.g. EPIC, Optum, MCCM, MCG, etc.) in a timely and accurate manner. 
Serves as site-based central point of contact for care management IT systems (e.g. MCG, Curaspan, etc.); contacts IT and/or vendor to report systems problems and to initiate troubleshooting and resolution of issues.




Education/Work Experience Requirements
Two years of related healthcare work experience that demonstrates the attainment of the requisite job knowledge skills/abilities required.
