  • JOB SUMMARY
    • Under direction of Bryan Health Resource Center Director, implements service coordination practices for the Bryan Health Resource Center. Promotes and supports wellness and community health to the general public, particularly to families in critical need, by connecting families with service providers, community resources, and other networks as necessary to serve client base. Engages clients through visits and follow-up communication. Develops relationships with community service providers and conducts research on community resources. Promotes the Health Resource Center to agencies and the public and engages in education on resource availability, utilization barriers, and unmet needs, Keeps documentation and produces reports. Undertakes skill development as needed. Coordinates with discharge planning and case management with local hospitals to assist with social determinants of health.

      • Required: High School graduate
      • Preferred: Bachelor's Degree

      • Required: N/A
      • Preferred: Education, health-related field or social work field
    • Experience
      • Required: 2 yrs. working in customer service field with direct client interaction;  experience in Microsoft Word, Excel, or other database management software
      • Preferred: 5 years of experience in community assistance, health or wellness programs, social services.

  • ACCOUNTABILITIES
    • ESSENTIAL
      • Implements service coordination practices for the Bryan Health Resource Center. Connects families with service providers, community resources, wellness programs, and other networks as necessary to service client base. Develops relationships with clients by conducting intake visits to gather information and complete assessments. Works with clients and their support systems to develop a resource access plan; monitors progress and follows up as necessary.
      • Develops relationships with community service providers and conducts research to discover and connect with community resources. Engages in a continuous process of education on resource availability, ways to overcome utilization barriers, and unmet needs.
      • Participates in select community groups or community associations with relevant missions. Promotes utilization of the Health Resource Center to agencies and the public by participating in informational expos, health fairs and other community events to increase awareness.
      • Keeps accurate records and produces reports. Uses database or other software tools developed by the Health Resource Center.
      • Undertakes skill development as needed to enhance the service provided by the Bryan HRC. (BVCAA energy assistance, ACA, Other entitlement programs)
      • Customer Service: Demonstrates customer service skills with both internal and external customers. Treats Patients/Customers as individuals with unique values, beliefs and practices.
      • Financial Accountability: Identifies opportunities for waste reduction and department efficiency.

      • Communication: Works as a team member demonstrated through interaction with others, assistance provided and behaviors exemplified.
      • Coordinates with discharge planning and case management with local hospitals to assist with social determinants of health.
    • OTHER
      • Performs other duties as assigned to meet the organization`s needs.
